Register Quickly for the Self-Advocacy Summit

The Calgary region has limited seating for the upcoming June 13-15, 2014 Self-Advocacy Provincial Summit. The Summit will be held in Edmonton at MacEwan University. 

You can download the registration form and send the completed application to the Calgary region by email or faxing it to 403.291.4087. 
Registration is due by May 16th, however seats are on a first come, first serve basis.

In the 10 page package,  all you need to send to us is page 5 and 6 (where you pick your workshops and fill out your contact and emergency information and sign the form). 

Please send in your filled out form as soon as possible as we are already half full. 

Once we receive your registration form, a confirmation letter will be sent at a later date.
